Bicholim: CHC Bicholim is all set to start their vaccination program for priority age group of 18 to 45 from tomorrow onwards at 9.30 am to 5 pm in their two vaccination centres – Shri Shantadurga High School and the multipurpose hall of Zantye college ,Sarvan.
In both the centres, around 200 beneficiaries will be vaccinated per day.
Following are the documents needed for inoculation of 18 to 44 priority group:
1) both parents who have child up to age two even lactating mother will be vaccinated.
(Need to bring birth certificate of child)
2) people with disabilities
(need social welfare certificate)
3) Seafarers
(need Continuing discharging certificate from captain of ports)
4) Taxi and rickshaw drivers
(RTO vehicular registration certificate and driving licence with rto batch )
5) Patients with following comorbidities
a) Diabetes
b) Hypertension
c) Cardiac disease
d) Chronic kidney failure
e) Cancer
(Need certificate from registered medical practitioner on their letter head containing their registration number of the respective councils. Even Ayush doctors certificate is valid)
